What You'll Do

Summary:
The Structural Heart Disease Program Manager will function as a central figure in the overall operations of the Heart Center with specific focus on structural heart disease management, including invasive procedures intended to treat patients with structural heart disease. Structural Heart diagnoses will include, but are not limited to heart valve disease, ASD/PFO, and atrial fibrillation. Responsibilities will include direct patient contact and coordination of care as well as the leadership of all team meetings and presentations. Specific responsibilities will include coordination of outpatient evaluations and follow-ups, procedure schedules and inpatient post-procedure care, monitoring of quality data bases and registries, quality and process improvement activities using LEAN methodology, oversight of documentation (creation of order sets, etc.) and compliance with all regulatory requirements associated with structural heart disease programs. The program manager will work in collaboration with a dedicated team of physicians, nurses, technicians and administrators, ensuring optimal care, outcomes and efficiency for patients with structural heart disease.


Responsibilities:
1. Develops the referral process and introduction of patients into the Heart Center for purposes of evaluation, assessment and potential treatment of structural heart disease. Responds to incoming referrals quickly to assure rapid access into the system.
2. Develops and leads an interdisciplinary care team (steering committee) for structural heart disease, and directs all program activity, including order sets, LEAN activities, quality and process improvement through this interdisciplinary team.
3. Guides and directs the continuum of care for structural heart disease patients from admission through discharge, including post-discharge care and follow-up as appropriate for this patient population. Conducts thorough assessments of the patient as needed. Manages multiple testing modalities to complete the assessment process.
4. Actively participates in the appropriate cardiac registries (i.e. TVT, LAAO) to assure that clinical data related to structural heart disease patients is reported accurately and appropriately for purposes of quality improvement. Provides concise and informative written reports and presentations of the data as requested by the Medical Director and the service line administrator.
5. Actively participates in any structural heart disease research projects that arise from the Heart Center as directed by the Medical Director and service line administrator.
6. Develops and implements protocols for care of structural heart patients in collaboration with an interdisciplinary care team (i.e. physicians, pharmacy, physical therapy etc.), which are evidence based. Provides program information and education as needed to interdisciplinary team and direct care providers. Researches and reviews current literature and clinical protocols and adapts for use at NHRMC.
7. Develops and implements the educational materials and support needed by the patient and family and assures that appropriate education and counseling are delivered to assist in progress toward established outcomes.
8. Communicates effectively, both verbally and in written format, the progress of the program and identifies areas for opportunity, growth and improvement to the medical and administrative team on an ongoing basis. Provides on a regular basis, updated program progress that includes outcomes related to all five Pillar goals, (Service, Quality, People, Finance and Growth).
9. Participates in performance improvement/research activities designed to improve patient care, and/or organizational processes.

Other information:
1. Education: Successful completion of an accredited school of nursing, radiologic technology or program of cardiovascular technology. Baccalaureate Degree preferred.
2. Licensure / Certifications: For nursing candidates, current license to practice nursing in the state of North Carolina. BLS and ACLS required. RCIS and/or CCRN preferred.
3. Experience: Five years’ clinical experience in cardiac critical care, cardiac surgical nursing or interventional cath lab experience. Related clinical experience will be evaluated and considered.
