What You'll Do


The Resource Specialist works in conjunction with the Population Health team to ensure enrolled patients are receiving the services they need to achieve optimal health. This position participates in community outreach and enrollment activities. Provides assistance/guidance to members, family members and caregivers. Serves on an inter-disciplinary team that provides care coordination, managing the medical, social, and behavioral health needs of enrolled members. The position requires a high degree of flexibility, independence and willingness to participate in multiple activities and provide support to all members of the team. The position must have strong communication skills and the ability to converse comfortably with patients and their caregivers, practice staff and program staff and administration. 


1. Participates in community outreach and enrollment activities for all identified patients/members. 2. Interviews candidates, family members and caregivers to compile and document relevant information (demographic, economic, social, institutional, etc). 3. Performs screening of candidates' eligibility for social services, quality, care management, and other population health programs. 4. Maintains required documentation in accordance with program policies and procedures and State/Federal regulations. 5. Describes available services and advantages of various care management activities. Provides program information and materials to candidates. 6. Participates in developing relationships with referral sources to facilitate enrollment. 7. Provides sensitive, culturally diverse assistance/guidance to patients, family members and caregivers. 8. Serves as a point-of-contact to program members, family members and caregivers. Informs and encourages use of program resources to maximize wellness and independence 9. Assists members in scheduling appointments for consultations, diagnostic testing, program meetings, etc. 10. In conjunction with the population health team, assists to arrange needed community services, such as transportation, DME 11. Collaborates with clinical and non-clinical team members to maintain proper workflows, procedures and documentation. 12. Participates in team meetings to provide non-clinical input regarding member status and activities. 13. Maintains integrity of related records in accordance with system policies and procedures and State and Federal regulations. 14. Develops and generates reports summarizing program activities, as required by management. 15. Performs related duties, as required. 16. Assists with special programs including data collection for process improvement projects. 17. Provide basic administrative function to support population health teams with coordinating information and materials with providers, patients, and internal/external customers. 18. Completes a variety of department specific tasks to assist with optimal department safety and functioning. 19. Appropriately communicates information in accordance with System policies and procedures. 20. Organizes and performs work effectively and efficiently. 21. Demonstrates positive interpersonal relations in dealing with all members of the team. 22. Communicates via electronic discharge system to obtain Medicaid authorization for patients discharging to SNFs. 

Position Requirements

1. Education: Graduate of two-year degree program or 2 years work experience commensurate with job 2. Licensure/Certifications: None 3. Experience: Two years of related work in healthcare environment is required, 4 years preferred. Must have sound knowledge of medical terminology. 

Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. Performs other duties as assigned. Individual will possess commensurate combination of education, experience and qualifications.
