Novant Health is hiring a home health registered nurse community liaison for the STD HRS shift for our New Hanover Regional Medical Center location in Wilmington, NC.

Schedule: Full-Time, 1.0 FTE, 80 Hours per 2 weeks, STD HRS Shift

About the Home Care - Skilled Nursing Department:

We’re seeking a registered nurse community liaison to join our team in our home health department. You will play a pivotal role in facilitating seamless transitions from the hospital to home care while serving as a liaison between healthcare facilities, patients, families, and the community. Your responsibilities will include acting as a liaison for the hospital, interacting with referral sources, hospital case managers, providers/staff in facilities, patients, and family members. Additionally, you will contribute to ensuring a smooth transition process and will utilize your marketing skills to engage with customers and the community effectively. 

Job Responsibilities: 

  • Engage with patients and their family members to assess their needs, provide education on home care services, and address any concerns or questions they may have. 
  • Building relationships within the community.
  • Educate physicians and their office staff and community of all services offered by NHRMC Home Care.
  • Assist in marketing plan development.
  • Coordinate with referral sources to assure a smooth process for admission of patients either directly from the physician office, facility or from home.

Job Qualifications: 

  • Graduate of an accredited school of nursing required.
  • Associate's or Bachelor's degree in nursing.
  • Current valid North Carolina nursing license required.
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required.
  • Driver’s License Class C.

If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
