Novant Health is hiring a Case Manager for the 8HR Day shift for our New Hanover Regional Medical Center location in Wilmington, NC.

Schedule: Full-Time, .7 FTE, 56 Hours per 2 weeks, 8HR Day Shift

About the Hospital at Home:

The Case Manager assumes a critical role within the healthcare ecosystem, partnering closely with physicians and fellow team members to orchestrate the seamless transition of patients from hospital to home. With a focus on proactive, complex, and safe discharge planning, they collaborate intensively to assess, strategize, implement, and assess discharge plans, ensuring optimal patient outcomes.

Job Responsibilities: 

  • Collaborate with the interdisciplinary team and patient/appropriate decision maker to develop an individualized discharge plan. 
  • Demonstrate assessment skills to collaboratively identify post-acute care needs. 
  • Communicate discharge plan to all healthcare team members, including the patient/appropriate decision maker and representatives from involved external agencies. 
  • Include patient/appropriate decision maker and interdisciplinary team in evaluating progress and modifying discharge plan to achieve desired outcomes. 
  • Develop and maintain positive relationships with all internal and external customers. 

Job Qualifications: 

  • Bachelor's degree of Social Work required. 
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required.
