Overview

Summary:

Reporting to and in collaboration with the Cardiac Services Administrator, this position is responsible for development and coordination of care programs for patients with all aspects of coronary artery disease. This includes but is not limited to community education, risk factor management, inpatient protocols and pathways, post-acute care and quality monitoring.

 

Responsibilities:

1. Establishes multidisciplinary ACS team and leads monthly meetings. Leads ACS team to develop and meet quality improvement goals/benchmarks. 2. Maintain in-depth knowledge of AHA/ACC guidelines for ACS, NSTEMI and STEMI. 3. Monitors and reports quality data from NCDR ACTION, Mission Lifeline and PCI registries. Collaborates with cardiac services administrator to develop appropriate quality improvement projects for ACS patient care based on review of outcomes data. Makes appropriate recommendations for project success. 4. Uses LEAN methodology as framework for quality improvement initiatives. Coordinates and leads A3 projects. Determines need for concurrent data collection, defect trackers, reports, etc., and works with support departments within the network to assure data is collected and reported appropriately. 5. Leads and reports on improvement initiatives for AMI readmission and AMI mortality. 6. Participates in Interventional Cardiology and Department of Cardiac Services monthly meetings, provides program status updates. 7. Facilitates collaborative relationships with all departments in the network that provide services to ACS patients. Routinely rounds in all areas. Involves women’s services in projects to increase awareness and treatment of women and heart disease. 8. Facilitates and participates in community projects related to ACS awareness and prevention, i.e. Heart Walk, Red Dress events, Cardiac Symposium. 9. Leads development of clinical pathways, protocols and order sets for all ACS patient groups in collaboration with physicians and other interdisciplinary team members. 10. Has responsibility for leading and maintaining quality designations i.e. CPC accreditation, ACC/AHA and Mission Lifeline awards. 11. Facilitates and maintains relationships with regional referral non-PCI hospitals regarding RACE program participation. Participates in RACE Committee meetings as needed. 12. Serves as resource to facilities in the region on a routine basis to assess and support the following: • Current state of ACS care. • Education and supporting materials regarding the AHA/ACC ACS/NSTEMI/STEMI guidelines • Support for development of ACS programs of care, to include prevention of CAD and community health. • Systematic changes likely to improve the likelihood and speed of therapy • Decision support from regional centers • Systems for rapid transfer for patients unable to be treated in primary hospitals. 18.Coordinate, participate in, and/or facilitate education surrounding the RACE intervention to each hospital in the RACE system. Travel to referral sites to conduct invited site visits and provide educational assistance. 19. Promotes positive relationships with EMS agencies and transfer hospitals in the region. 20. Recommend changes and modifications to systems as required to ensure timeliness, accuracy, and efficiency. 21. Participate in and encourage your system’s attendance at all related RACE PCI meetings. 22. Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.) 23. Performs other duties as assigned.

* Bachelors Degree

Other information:

1. Education: Bachelor’s degree required (BSN, BS, BA); or Education agreement to obtain Bachelor’s within 1 year. Strongly prefer candidates with a master’s degree, either in Nursing, Science, or Health Care Administration (MSN, MS). 2. Licensure / Certifications: State of North Carolina RN licensure, Registered Radiologic Technologist, (RT-R) RCIS or Licensed Paramedic BLS/ACLS required. (ACLS may be completed within 90 days of initial hire, maintained thereafter.) 3. Experience: Minimum 5 years of related clinical or program experience that includes care of acute cardiac patients.
