What You'll Do

Summary:

The MDS Coordinator reports to the Manager, Skilled Nursing Facility. Responsible for MDS assessments ensuring all state requirements/regulations are met.


Responsibilities:

1. Provides education & information on the MDS process to employees as needed. 2. Serves as resource person for resident planning. 3. Completes individual resident care plans & makes timely changes as needed. 4. Completes MDS assessment in a timely manner and communicates as needed to director/therapies. 5. Support Program Development Activities. 6. Promotes optimal communication among SNF staff members, ancillary staff, referral sources, physicians, residents and family members. 7. Attends and participates in department/facility meetings as required. 8. Participates in direct and indirect patient care as warrants (i.e. covering staffing issues as needed) 9. Transmits MDS assessments to the state data base in a timely manner. 10. Weekly weight loss review and care plan team. Provide staff with updated weekly and monthly printed forms for weight monitoring. 11. In charge of facility in absence of Manager & Clinical Coordinator.

Credentials:
Essential:
* Licensed Practical Nurse
* Registered Nurse

Education:
Essential:
* Associate Degree

Other information:

Education: Graduate of a Board of Nursing or NLN accredited school of nursing. 

Licensure/Certificate: Current licensure as a LPN or RN by the NC Board of Nursing or compact state. CPR certification. 

Experience: At least one year long term care and MDS knowledge preferred. 

Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
Individual will possess commensurate combination of education, experience and qualifications.
