In partnership with the Clinic Administrator, ensures implementation and success of NHMG goals, organizes and directs the clinical environment, and coordinates teams to assure effective care is given to provide quality patient care in physician practices. Supervises between 10 and 19 FTEs. The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, delegate and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death and takes ownership in creating an environment that allows team members to be present with patients and families. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures. The Registered Nurse gives feedback and evaluations to team continuously to monitor appropriate team members for positions and promote employee and customer satisfaction.
Essential Functions • Leadership: Works in partnership with other leaders to create, support and maintain a culture of professional accountability. Utilizes the ANA Scope and Standards of Practice, Code of Ethics for Nurses with Interpretive Statements, evidence based practice and guidelines from formal nursing organizations as the foundation for nursing practice. Meets and maintains requirements of a Registered Nurse and guidelines from formal nursing organizations as the foundation for nursing practice. Assures that team members provide care that is consistent with The Joint Commission and other regulatory requirements. Actively participates in team meetings and promotes collegial relationships with physicians and other professional groups. Removes barriers for staff to assure optimal patient safety and satisfaction. • Assessment: Demonstrates refined assessment skills to collaboratively discover the patient's story, integrating the following elements: subjective data, objective data, physical picture, psychosocial components, personal preferences and norms, and learning needs and preferences. Must be able to utilize an ongoing process to interpret data and recognize patterns and trends to determine both immediate and future care needs. • Planning: Dialogues with the patient/chosen support system to identify what is important to them and incorporate his/her preferences and norms into the individualized care plan. Engages the interdisciplinary care team in planning individualized care. Identifies relevant resources based on necessity and availability. Determines immediate needs and establishes realistic short term and long term goals based on desired outcomes. Prioritizes interventions to advance the patient toward optimal health or support a peaceful death. Constantly considers ongoing modification of care plan based on patient's needs, capabilities, and condition. • Implementation of Interdisciplinary Plan of Care: Provides age-specific patient care by assessing, planning, implementing, evaluating, and documenting the individual patient needs, actions taken and the patient responses relative to nursing care. Accepts direction from provider. Reviews orders for clarity and appropriateness. Acts with patient advocacy at the core of nursing care when implementing the interdisciplinary plan of care. Collaborates with healthcare team to educate patient and their support system. Identifies patient learning needs, incorporates into care by providing education and counsel to patients, and documents patient understanding. Analyzes, modifies, and implements strategies to progress the patient through their plan of care. Ensures completion of nursing interventions, medical orders, and other related therapies, using approved policies/procedures/protocols. Possesses the ability to prioritize job duties according to patients' needs and changing acuity levels. Performs patient transfers and transports patients via wheelchair. Ability to lift and carry various equipment. Ability to provide routine nursing care such as start IVs, perform dressing changes, etc. for multiple patients in a timely manner. Ability to perform in critical situations including administering CPR. Helps to facilitate care provided is consistent with policies, procedures and telephone triage protocols, in cooperation with the physician. Able to function in emergency situations and facilitates the care provided for all patients. Makes patient referrals as directed by the provider, and as appropriate, to resolve identified problems or reach established patient goals. The role involves working in a team based patient centered environment. Evaluation: Continuously assesses patient's progress toward goals and outcomes. Seeks input from patient and their chosen support system regarding progress toward goals. Analyzes and modifies plan of care with patient and interdisciplinary team as indicated to achieve desired outcomes. • Communication and Documentation: Communicates individualized plan of care and patient’s progress toward goals to all members of the care team including the patient and his/her chosen support system. Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care. Documents appropriate information in a timely manner. • Delegation: Appropriately delegates to ensure the implementation of the plan of care while ensuring patient safety, considering the nature of the task, competence of the delegate, and the patient's response. Demonstrates commitment to quality patient care by holding self and others accountable for optimal role performance. Delegates or assigns patient care to appropriate health care team members commensurate with their validated competency to perform the task, evaluates outcomes. Supervises, teaches clinical team, and provides feedback, as needed. Assists in orientation of new team members and serves as a resource person and professional role model, within scope of practice. Assists with assuring competencies of clinical staff. • Performance Accountability/Evidence-Based Practice: Demonstrates independence in the foundation of nursing care practice within the specialty. Practice according to your state's Nurse Practice Act, ANA Code of Ethics and ANA Bill of Rights. Identifies opportunities for process improvement, incorporates evidence-based practice and research. Engage in self-reflection and develop personal goals to enhance knowledge, obtain professional certification or pursue academic degree as strategies to promote professional growth. Inspires team members and helps connect the work of nursing to the goals of the organization • Clinical Lab Duties (where applicable): Specimen collection and Testing Analysis: Performs venipuncture, skin puncture and lab procedures with attention to accuracy by checking patient ID and DOB. The employee understands and follows HIPAA guidelines and ensures the confidentiality of patient results. Reporting results: Ensures all test results, in-house and referred, are reported to the provider by checking a manifest or providing oversight of pending labs. The team member documents or assists in documentation or corrective action needed for Quality assessment, Proficiency Testing and Quality Control. Documentation: Ensures all routine, daily, monthly, and annual maintenance and QC are performed, documented and kept for the appropriate number of years. • Non-Clinical Duties: Records employee’s time and attendance. Records employee’s tardiness and absentee. Follows-through with the Practice Manager’s staffing plan. Maintains inventory of medical/office supplies and order medical/office supplies. Monitors and records refrigerator temm during clinic hours. Observes team members for customer service standards. Supports an environment that respects diversity. Interacts with patients/families daily consistent with the state nursing practice act. Provides accurate and consistent communication. Participates in team member meetings. • Quality Improvement: Collaborates with the clinic administrator and physician to recognize and communicate opportunities for improvement related to best operational efficiencies in the physician practice setting. Embraces change and acts as a change agent. Monitors quality of completion of core processes in the department. • Human Resources and Organizational Development: Participates in the interview process. Participates with clinic administrator to maintain employee human resource files based on corporate policy. Attends educational offerings related to his/her competency • Human Experience: Provide guidance to team members as they navigate patient concerns through teaching, good communication and trust building behaviors. Seek feedback from team members for innovative ideas to improve the experience for both patients and team members. Utilize relationship building skills through rounding to provide training and coaching to team members for how to effectively connect with patients. Provide guidance, training and coaching to team members on Always Event behaviors and use patient feedback to create innovative ideas to meet patient needs. Acknowledge the diversity of your team and patients and model an inclusive environment by involving others in decision making.
