What You'll Do

Summary:

The employee’s Number One job responsibility is to deliver the most remarkable patient experience, in every dimension, every time, and understand how he or she contributes to the health system’s vision of achieving that commitment to patients and families. The Supervisor, Community Health Services is responsible for the oversight of day-to-day operations of community health services and outreach related initiatives that support system efforts to address social determinants of health and access to care. He/she will assist Novant Health team members in key departments to integrate best practices in providing community based care for vulnerable populations: this includes collaborating with multi-disciplinary teams such as care connections, Novant Health Medical Group, Value Based Care, Patient advocacy and Diversity and Inclusion. He/she will act as a field liaison and educator to social work interns as well as providing supervision to Health Services Community Events team. Supervisor will also assist with program development, strategy, and implementation as need. The Supervisor of Community Health Services must possess and demonstrate effective application of leadership, relationship building, decision-making, execution and accountability to ensure the continued institution of best practices regarding the integrity of Novant Health’s integrated community benefit outreach strategy. 


Responsibilities:

Leadership: Embraces change necessary to improve efficiency and/or effectiveness and creates an environment for change by empowering team members with resources necessary to address social determinants and integrate key community benefit strategies into the framework of Novant Health care delivery processes. Acts with honesty and integrity showing and speaking with respect for the rights of others, human dignity, local culture and about Novant Health’s mission, vision, and values. Supports system changes from a social determinant perspective to aid Novant Health’s mission of improving the health of communities. Demonstrates leadership skills including problem solving, delegation, time management, and organization. Provides oversight for community health services initiatives and programs. Focus on collaborative partnerships to optimize measurable outcomes. Ensure Novant Health is appropriately represented in the community. Supervisory: Ensure Mobile Cruiser staff are working efficiently and effectively in their roles in the community. Communication: Actively listens, communicates, and engages multi-disciplinary teams to improve systems for care delivery. Ensures timely delivery of information delivered to appropriate audience. Ability to develop and maintain strong relationships with Novant Health leadership team, team members, and local community agencies. Ability to work with local community agencies and use creative problem skills while keeping the patient at the center of all decisions. At all times, interactions with team members will be on a professional level. Maintains the ethical standards of NASW code of ethics. Quality Improvement: Consistently reviews core processes, looking for improvement opportunities. Embraces change and acts as a change agent. Maintains and adjusts schedule to enhance team performance. Project Management and Administration: Responsible for all cruiser operations to include daily schedule and care on cruiser. Work with leadership and team members to develop drafts for project deliverables such as project plans, schedules, budgets, etc. Lead and manage small to medium sized projects. Responsible for follow-through on all aspects of the assigned projects throughout the project lifecycle. Ability to provide timely and accurate project statuses and decision support information. Coordinate meetings and follow up with other key departments involved in community outreach and community benefit activities to establish and organize priorities, demonstrate initiatives, and execute closures and handoffs with appropriate timelines. Reports project process against measurable outcomes. Ensures all project and program deliverables are kept current, accurate, and published on technology platforms and distributed as required. Consolidates and maintains project and program plans, status reports, creates and maintains resource plans. Creates resources, guides, reference materials, presentations, etc. as identified to enhance transparent and consistent messaging. Professional Growth and Development: Attends continuing education annually to stay abreast on best practices related to community health improvement services, social determinants of health, behavior change methods, and innovations in care for the underserved. Education: Bachelor's Degree Required 

Experience: Minimum of three years experience in health care setting. Or community health Required 

At least 1 year of leadership experience Preferred 

Licensure/Certification/Registration: Registered Nurse Preferred BLS is required 

Additional Skills Required: This position has access to and knowledge of extremely sensitive, private, and confidential materials. Ability to maintain the high standard is required with zero tolerance. Ability to work on a variety of partnerships and projects concurrently. Ability to effectively communicate with peers, diverse stakeholders, and Novant Health team members at multiple levels of the organization. General working knowledge of community health improvement strategies. Experience in Word, Excel, PowerPoint, and Dimensions/EPIC software. Lifelong learner with knowledge of community resources. Travel for business purposes is required (minimal). Must poses a valid driver’s license and reliable insured vehicle. Flexible work schedule to include some nights and weekends.
