Work Schedule:

  • Tuesday – Friday, 8:30am – 5:00pm

  • Saturday, 11:00am – 7:30pm

What You'll Do

Summary:

Our discharge analysts maintain record integrity and supports patient care and billing process. Works under the supervision of the Manager of Health Information Management, performs quantitative and quantitative analysis on electronic medical records, noting deficiencies and assigning them to the appropriate physician in Electronic Medical Record (EMR) for completion. Expedites priority accounts to achieve an optimal accounts receivable goal. Reanalyzes medical records to determine that deficiencies have been completed, updating deficiency status in EMR.


Responsibilities:

1.Promptly performs quantitative and qualitative analysis of documentation requirements on electronic medical records, identifying all deficiencies in EMR to be completed by physicians and other health care professionals. Assist Physicians and Providers in completion of deficiencies in the electronic completion.

2.Demonstrates knowledge of medical record rules and regulations

3.Promptly performs validation and reanalysis of medical records to ensure completion and removes deficiency from EMR. Ensure the complete and accurate record completion in accordance with compliance with regulations

4.Responsible for weekly communication and notification to Providers of delinquent deficiencies. Created list of providers who did not complete delinquent deficiencies within policy or procedure. Records and tracks communication/record keeping of delinquent records.

5.Actively communicates with physicians assigned to deficiencies to eliminate delinquencies and encourages timely completion. Interacts and communicates with providers, and their office staff, registration staff, HIM systems analyst, chart correction, and prepping scanning and indexing staff to coordinator the deficiency process.

6.Analyzes records returned to work queue as a high priority.

7.Meets assigned productivity standards according to department policies

8.Staff is productive in meeting assigned tasks and job duties.

9.Consistently works towards the analysis completion time frames established by management.

10.Assists with basic care and maintenance of workstation equipment.

11.Maintains cooperative and harmonious relationships with professional staff.

12.Maintains cooperative and harmonious relationships within the department, coworkers, and other areas of contact.

13.Participates in the total quality management efforts of the department.

14.Fosters respect for patient privacy by maintaining confidentiality in all phases of work.

15.Completes productivity workbook daily.

16.Performs coding support functions as assigned to include discharge disposition verification, missing reports and have them scanned into OnBase.

17.Review demographic data for all portions of the record that are analyzed and report errors identified to manager via epic in basket.

18.Collaborates with Data integrity and other HIM staff. Ensures errors in documentation discovered are communicated for Chart Correction.

19.Research physician rejections to determine correction deficiency assignment.

20.Must demonstrate the ability to search out and verify orders, notes, and results for inpatient, observation, ASC, Emergency room, outpatient documentation in the electronic medical records.

Position Requirements


  • Education: High School diploma required with at least 2 years of direct or related work or educational experience, Prefer experience with Electronic Medical Record. Basic knowledge of medical record content and documents and knowledge of medical terminology preferred.
  • Licensure/Certifications: None
  • Experience:  Two years of direct or related work or educational experience is required.  Two years in a Medical Records Department preferred. Advance problem solving and analytical skills. Detailed knowledge of medical record content and chart organization. Must possess knowledge of medical terminology. Demonstrates skills necessary to use computer software as job duties dictate to include but not limited to EPIC, OnBase, CPSI and Legacy systems

Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. Performs other duties as assigned. Individual will possess commensurate combination of education, experience and qualifications.
