Work Type: 64 to 80 Hours Pay Period

Work Schedule: 8HR DAY

What You'll Do

Summary:
Works in collaboration with the Chronic Care Management and Population Management staff to provide exemplary patient care and to implement and manage electronic plans of care for high-risk patients to improve the quality of life and minimize hospital readmissions and emergency room visits. Document patient telephonic interactions which may include medication management, results of laboratory or radiology tests and reports as directed by physicians, as well maintain preventive and disease management services. Assists in administrative duties as well as clinical.

Responsibility I: NHRMC Standards of Performance

Responsibility II: Clinical
• Work from epic dashboards to identify high risk patients
• Establish open patient communication using patient outreach encounters
• Work with provider and patient to establish personalized patient goals then work with patient toward achieving those goals
• Create electronic ongoing plan of care specific to patient’s needs
• Establish patient’s social, safety, financial, and fitness status so to understand and develop appropriated rapport for ongoing health care communication
• Verify all patient referrals are kept, emergency room visits have follow up with provider
• Give patient follow up care instructions as directed by provider
• Maintain chronic care management minute tracking through out the month and drop appropriate charges as directed.
• Assist patient in dietary monitoring and home glucose monitoring when applicable
• Assist patient with the scheduling of healthcare services or specialist referrals
• Assist patient with appropriate referral to community resources
• E-scribing prescriptions to pharmacy as directed by physician
• Use communication skills using appropriate medical terminology
• Following appropriate legal and ethical professional conduct

Responsibility III. Clinical

• Recording patient interview and history
• Strong working knowledge of current medications, dosing information and prescribing requirements
• Providing patient education with regards to office policies, medications, management of diseased, home treatments and special diets
• Capable of accurate, legal, and ethical documentation at all times
• Maintains compliancy within regulatory agencies such as HIPAA, OSHA and CLIA and JCAHO if applicable

Responsibility IV: Accountability of Patient Care
• Ensures that patients have all information and questions answered about their health and treatments received, to include future treatment plans
• Completes all documentation for observation, planning and evaluation of care provided to patients
• Assumes responsibility for tracking patient through radiology, laboratory and all other referrals

Responsibility V: General Practice
• Must have physical exam and annual TB skin testing
• Provide telephone coverage / daily patient call backs
• Capable of scheduling future appointments and referrals as necessary
• Refrain from discussing Patient Health Information in public in accordance with HIPAA guidelines and policies
• Ability and flexibility to cover various positions in CCM department
• Adheres to departmental dress codes as observed by director and wears picture identification badge, 100% of the time

Education:
Essential:
* High School Diploma

Other information:
1. Education: HS diploma or equivalent required.
2. Licensure / Certifications: CPR Certification Preferred
3. Experience: 3 years previous medical / physician office experience preferred.
Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
Individual will possess commensurate combination of education, experience and qualifications.
