Full Time Equivalent: Fulltime

Work Type: 64 to 80 Hours Pay Period

Work Schedule: 8HR DAY - Most of shift btwn 7A-3P

What You'll Do

Summary:

Works under the direction of the Manager of Case Management. Works to insure the establishment of a home support system or alternative living placement plan which is realistic in relation to the patient’s present circumstances and facilitate positive outcomes in collaboration with the healthcare interdisciplinary team and available community resources with a focus on enhancing the capacity of people to address their own needs. Provides support and crisis intervention for patients and significant others in relation to such issues as domestic violence, advanced care planning, adoption, financial resourcing.


Responsibilities:

1.Utilizes strong psychosocial skills to facilitate developmentally appropriate patient assessment, plan of care, interventions, and evaluation processes.

2.Performs psychosocial assessment as needed.

3.Provides counseling, support and crisis intervention for patients and significant others, coping with difficult situations including intervention for abuse, neglect and domestic violence involving patients and families.

4.Facilitates the adoptive process and release of children taken into custody.

5.Assesses the need for guardianship and facilitates the guardianship process.

6.Assists with the commitment process including transportation of patients to out of area facilities.

7.Acts as Hospital Liaison to the Department of Social Services for protective service cases.

8.Facilitates/Assists patients/families with advanced directive planning to assure that “Advance Directive” & “Health Care Power of Attorney” forms have been filled out correctly and notarized.

9.Utilizes complex decision-making and solving skills that facilitate developmentally appropriate patient outcomes.

10.Assesses the patient’s financial needs and facilitates connecting them with community resources available.

11.Acts as an advocate for the patient to ensure their needs are met and their rights are protected.

12.Assists with establishing a home support system or alternative living placement plan, which is realistic in relation to the patient’s present and potential function, emotional, and developmental capabilities and is attainable in relation to the human, equipment, and community resources available (ie. Homeless, lack of family support, etc).

Credentials:

Education:
Essential:
* Bachelor of Science

Other information:

Education: Bachelors Degree in Social Work or related Human Services Field.

Licensure / Certifications: North Carolina Social Work Certification preferred. American Heart Association BLS required.

Experience: Minimum of 2 years experience in a health care setting.

Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
